Trichinellosis Outbreak in North Carolina Linked to Undercooked Bear Meat

In a recent incident in western North Carolina, an outbreak of probable trichinellosis cases has been linked to the consumption of undercooked bear meat. Trichinellosis, also known as trichinosis, is caused by ingesting live Trichinella parasites that are commonly found in undercooked meat, particularly bear meat. The outbreak, involving 10 probable cases, was reported in the latest Centers for Disease Control and Prevention Morbidity and Mortality Weekly Report.

The North Carolina Division of Public Health first became aware of the outbreak when a person experienced flu-like symptoms and facial swelling after attending a gathering where undercooked bear meat was served. Further investigation revealed that 65% of the attendees had consumed the bear meat, with 45% subsequently showing clinical signs and symptoms of trichinellosis. Symptoms included facial swelling, muscle aches, and fever, with onset occurring between seven to 26 days after consumption.

Due to limited information and lack of confirmatory testing, the cases were classified as probable trichinellosis. Most patients had received treatment with albendazole, an anti-helminthic medication. The ages of the patients ranged from 10 to 40 years, with six of them being 18 years old or younger.

Trichinellosis can have serious consequences if left untreated. The parasite larvae can burrow into the walls of the small intestine, leading to symptoms such as diarrhea, abdominal pain, fatigue, and nausea. As the larvae migrate through the bloodstream and into the muscles, individuals may experience muscle pain, weakness, and inflammation, which can be debilitating.

In rare cases, trichinosis can result in severe complications such as myocarditis, pneumonitis, encephalitis, or meningitis, which can be fatal. Treatment with albendazole is recommended to eliminate the parasites, although it can be costly.

To prevent trichinellosis, it is important to avoid consuming raw or undercooked bear meat, as well as meat from other wild animals such as cougar, walrus, fox, wild boar, or pig. Meat should be cooked thoroughly to an internal temperature of 165°F to ensure that any parasite larvae are killed. Additionally, proper hygiene practices should be followed when handling raw meat to prevent contamination.
