In 2011, two key events marked the beginning of a new era for community pharmacy. The first was a public dispute between Walgreens and Express Scripts, one of the nation’s largest pharmacy benefit managers (PBMs). The dispute centered on reimbursement rates, with Walgreens taking a bold stand against what it saw as unfair compensation. The company’s refusal to accept the terms set by Express Scripts was framed as a moral and economic stand for fair treatment. However, the underlying issue was the shifting perception of pharmacies as interchangeable distribution points—a commodity business rather than a trusted service provider.

The second significant event in 2011 was the announcement of the merger between Express Scripts and Medco, creating a PBM powerhouse with unprecedented leverage over pharmacies. The merger shifted the balance of power in the drug supply chain decisively towards PBMs, further squeezing the margins of traditional retail pharmacies.

The 2010s brought a wave of disruption and disintermediation to the pharmacy industry. Online retailers like Amazon and Target.com began to erode foot traffic to traditional pharmacies’ front of store businesses, while innovators like PillPack reimagined pharmacy as a logistics problem rather than a retail one. These new pressures, combined with the relentless margin compression imposed by PBMs, pushed many traditional pharmacies to the brink of collapse.

Despite these challenges, there may still be a glimmer of hope for the future of community pharmacy. As the industry continues to adapt to changing consumer behavior and technological advancements, there is an opportunity for pharmacies to reinvent themselves and reclaim their role as trusted healthcare providers in the community. By embracing innovation, collaboration, and a renewed focus on patient care, pharmacies can navigate the turbulent waters of the modern healthcare landscape and emerge stronger on the other side. The acquisition of PillPack by Amazon in 2018 marked a significant shift in the pharmacy industry. It was not just about acquiring a mail-order pharmacy, but also about obtaining a user experience and a supply chain playbook. With new entrants like Capsule and Alto promising home delivery with digital convenience, the landscape of pharmacy was rapidly evolving.

The healthcare reform and expansion of the 340B program after the Affordable Care Act further tilted the economics away from community pharmacies. Originally intended to provide financial support for safety-net providers, the 340B program required drug manufacturers to provide steep discounts to select providers. However, with the expansion of the program, critics pointed out that large hospital systems were capturing a growing share of drug margins through 340B pricing, further fragmenting the market.

By the late 2010s, pharmacy chains were optimizing for survival by cutting costs, reducing labor hours, and automating processes. While metrics like more prescriptions filled per store and higher productivity looked solid on paper, something essential was missing – profit.

The decline of retail pharmacy was not due to operational failure. Data showed that independent pharmacies were dispensing more prescriptions and growing revenue over the past decade. Walgreens, CVS, and other chains became more efficient, productive, and data-driven than ever before. However, the core economics, specifically the gross margin, collapsed.

The gross margin in pharmacy, defined as revenue minus the cost of goods sold, had significantly decreased over the years. Pharmaceutical manufacturers enjoyed high margins, while retail pharmacies saw their gross margins fall under 20%, and continuing to decline. This collapse in gross margin impacted the capacity for reinvestment in growth and innovation, leading to a suffocation of the enterprise.

The generic wave, which initially benefited pharmacies as blockbuster drugs lost patent protection, turned out to be a mirage. As more manufacturers entered each generic market, competition drove acquisition costs down, eroding margins for pharmacies. The shift to 90-day prescriptions and the promotion of mail-order pharmacies further exacerbated the challenges faced by retail pharmacies.

Despite these challenges, there are signs of renewal in the pharmacy industry. Pharmacies have proven their public health value through services like influenza vaccinations and COVID-19 vaccine administration. Policy shifts at the state level are granting pharmacists more authority to bill for clinical services and prescribe medications for select conditions. Technology is also enabling this shift, with companies developing tools to support pharmacists in providing value-based care and improving patient engagement and fulfillment.

Overall, while the pharmacy industry faces significant challenges, there are opportunities for renewal and innovation that could reshape the future of pharmacy practice. National interoperability efforts, like The Sequoia Project’s Pharmacy Interoperability Workgroup, are actively working to enhance the connectivity of pharmacies within the broader healthcare data ecosystem. This initiative aims to integrate pharmacies more fully into the healthcare data landscape, ultimately creating a more cohesive and streamlined approach to patient care.

The evolving landscape of pharmacy suggests a potential shift towards a distributed primary care network that complements traditional physician-based care rather than competing with it. This transformation highlights the increasing importance of pharmacists in managing chronic conditions, titrating medications, and improving medication adherence. By empowering pharmacists to take on a more active role in patient care, the healthcare system can potentially improve access, affordability, and overall patient outcomes.

However, in order to scale this vision for the future of pharmacy, it is crucial to address the underlying structural challenges that have led to the decline of community pharmacies. Pharmacy Benefit Managers (PBMs) have played a significant role in shaping the current landscape of pharmacy by focusing on cost management. While PBMs have been successful in moderating drug spending growth, their emphasis on efficiency has also led to the closure of many community pharmacies, resulting in decreased access to care.

The question at hand is not just how to regulate PBMs, but rather what we envision community pharmacy to be in the future. Should pharmacists be empowered to take on a more proactive role in patient care and be compensated accordingly? While some stakeholders, like the American Medical Association, have raised concerns about expanding the scope of practice for pharmacists, maintaining the status quo is not a sustainable solution.

In order to revitalize and strengthen the role of community pharmacies, it is essential to recognize the value that pharmacists bring to the healthcare system. By leveraging policy changes, technological advancements, and innovative practices, stakeholders can work together to ensure that pharmacists continue to serve as integral members of the healthcare team.
