Planning for the end of life is not something most people enjoy thinking about. However, it is essential to have a plan in place to ensure that your wishes are honored when the time comes. Many experts recommend documenting your preferences clearly and unambiguously in your medical records so that your doctors can refer to them in case of mental incapacity.

One common approach is to create a living will, which allows you to outline specific medical interventions you do or do not want in certain situations. For example, you can indicate whether you would want to receive CPR, be put on a ventilator, or receive artificial nutrition and hydration. While this approach can be effective for some people, it may not work for everyone.

One concern with detailed advanced directives like living wills is that they ask you to imagine scenarios you may have never experienced and make decisions without the full context of your future health. Additionally, these documents can potentially limit the flexibility of your family and healthcare providers to make decisions that they believe are in your best interests.

A study conducted at Johns Hopkins University found that patients who participated in advanced care planning were more likely to document their end-of-life preferences but also more likely to receive potentially burdensome care near the end of their lives.

An alternative approach is to designate a trusted individual to make decisions on your behalf if you are unable to do so. This person should be someone who knows your values and preferences but also has the autonomy to make decisions based on the specific circumstances at hand. By trusting this individual to act in your best interests, you can ensure that your wishes are respected while also allowing for flexibility in decision-making.

It is important to have these conversations with your loved ones and healthcare team to ensure that everyone is on the same page regarding your end-of-life care preferences. While it may be uncomfortable to think about these scenarios, taking the time to plan ahead can provide peace of mind and ensure that your wishes are honored when the time comes.
