The FDA has withdrawn a lab test result related to the ongoing cyclospora outbreak associated with Taco Bell lettuce, leading to public uncertainty about the situation.
As someone who has served as an Epidemic Intelligence Service Officer with the CDC, I have insight into the complexities of outbreak investigations. While a single lab test might not detect the parasite, this doesn’t necessarily imply that health experts are incorrect. One data point, such as a negative test or its retraction, is not enough to undermine the entire investigation.
Why Outbreak Science Is More Than One Test
Food outbreak investigations rely on a long-established and rigorous method to pinpoint the source. This has been effective in resolving numerous outbreaks, such as the 1993 Jack in the Box E. coli outbreak linked to undercooked meat, the 2006 spinach E. coli outbreak in California, the 2011 cantaloupe listeria outbreak from Jensen Farms, and the 2018 romaine lettuce E. coli outbreak traced to Yuma, Arizona.
Lab tests are crucial in identifying outbreak sources, but the initial clues often come from affected individuals’ reports. Investigators cast a wide net, interviewing many people about their experiences to identify timeframes, locations, and exposures to particular foods.
Once data is gathered, investigators search for commonalities among the sick individuals. This process can be painstakingly slow and requires sifting through vast amounts of information to find relevant clues.
The Role Of Qualitative Data In Tracing Outbreaks
The FDA lab test incident brings to mind a past investigation I conducted in Indiana. There, lab results conflicted with information from the health department, leaving the source unknown. The breakthrough came not from advanced DNA technology but from listening carefully to people’s accounts until the transmission path was revealed.
Outbreak investigations require both quantitative data and qualitative insights. While one aspect might prove inaccurate, it does not negate the other.
These investigations are not without challenges. They can involve trial and error, misinterpreted clues, and false leads before a breakthrough occurs. The current task is to identify which Taco Bell item is spreading cyclospora.
In this instance, Michigan health officials spoke to 190 Taco Bell customers who fell ill; 90% of them consumed iceberg lettuce. This is a credible result that stands on its own, regardless of lab test results.
Solving foodborne outbreaks demands time, dedication, cooperation, and openness. This case will be resolved, and the public is encouraged to be patient and trust the scientific process that has consistently delivered results. The answer should be forthcoming soon.
