On Wednesday, the Trump administration introduced a new initiative aimed at addressing the interconnected issues of addiction and homelessness in the United States.

The White House released a new toolkit featuring a “treatment first” model that emphasizes faith-based interventions, departing from the harm reduction and “housing first” strategies previously adopted by the Biden administration. This approach prioritizes medical treatment for addiction, with abstinence as its ultimate goal, rather than acknowledging reduced drug use and related harm as positive outcomes.

This toolkit aligns with the Trump administration’s historical stance on addiction, which includes the Great American Recovery Initiative and a “tough love” policy toward drug users, alongside a general opposition to harm reduction.

Health Secretary Robert F. Kennedy Jr. stated, “Treatment must come first. Recovery begins when we connect people with effective care, not leave them trapped in addiction and homelessness.” He added, “This toolkit provides communities with practical, evidence-based strategies to expand treatment, support lasting recovery, and help people reclaim their independence.”

The program advocates a comprehensive approach to addiction recovery and homelessness, suggesting consistent sleep patterns, daily routines related to treatment, and employment as key components of its strategy.

The administration made it clear they do not support unconditional access to housing, citing a study indicating that veterans in supportive housing were more likely to die from a drug overdose. Abstinence remains a primary objective, though the toolkit acknowledges that relapse is a setback rather than a failure, and recovery is not always a linear process.

Scott Turner, Secretary of the Department of Housing and Urban Development, emphasized, “I have met men and women whose lives have been transformed by treatment and recovery. Housing is essential, but housing alone is not enough.”

Despite opposition from the Trump administration, “housing first” strategies have been successful, notably in Houston, where the city relocated 25,000 people from the streets into homes over a decade. City officials describe the program as successful locally and nationally, though it faces funding challenges while homelessness rates remain unchanged.

The strategy also advocates for employing individuals with personal experiences of addiction, homelessness, and recovery in influential roles within organizations focused on homeless outreach and recovery support.

While the strategy makes only brief mention of methadone and buprenorphine—medications that significantly reduce the risk of opioid overdose—a series of federal actions have raised questions about the administration’s commitment to medication-assisted treatment for opioid use disorder.

Although the anti-harm reduction stance may concern some advocates, there is a favorable view of the administration’s plan for addressing meth addiction.

The document notably endorses contingency management, which involves providing financial incentives to individuals who reduce or cease drug use. This approach aligns with current evidence, though some providers feared it might clash with the administration’s ideology.
