The America First Policy Institute has recently published an issue brief shedding light on a crucial issue—that the rest of the world benefits from American pharmaceutical innovation at the expense of American consumers. This unfair burden must be addressed by the Trump administration, just as the President has taken a firm stance on other forms of foreign freeloading.

The AFPI correctly identifies foreign freeloading as a problem that negatively impacts American patients and global research efforts. However, their proposed solution of implementing a “most-favored-nation” (MFN) pricing policy is fundamentally flawed. This approach fails to consider the intricate negotiations and price controls that European countries use to secure discounted prices from pharmaceutical companies.

Implementing an MFN policy would not only fail to address freeloading but also risk importing Europe’s price controls into the American healthcare system. This could severely hinder American innovation in the pharmaceutical industry, as developing a new drug is a costly and time-consuming process with no guarantee of success.

Research has shown that reducing pharmaceutical prices in the U.S. could lead to a significant decrease in research initiatives. Additionally, adopting European-style price controls could have resulted in the loss of over 100 new medicines in the past few decades.

Furthermore, the majority of new drugs are first introduced in the United States because the market compensates pharmaceutical companies fairly for their research and development costs. Lowering American prices to match European levels would only exacerbate global freeloading and potentially shift medical innovation leadership from the U.S. to countries like China.

Instead of resorting to price controls, the Trump administration should leverage trade tools and diplomatic channels to ensure that foreign countries pay fair prices for American-developed medicines. By directly confronting foreign governments, the U.S. can protect its innovation, maintain patient access to groundbreaking treatments, and retain its position as a global leader in medical research.
