Erythritol, a common sugar substitute often touted as a “healthy” option, has been linked to potential health risks in a recent study presented at the 2025 American Physiology Summit in Baltimore. This flagship annual meeting of the American Physiological Society shed light on the impact of erythritol on blood vessel health, highlighting potential concerns for consumers.

Found in a variety of sugar-free products such as energy drinks, snack foods, and protein bars, erythritol is favored for its low-calorie content and minimal effect on blood glucose and insulin levels compared to sugar. However, previous research has raised red flags regarding its association with adverse cardiac events, including stroke.

The study conducted by researchers involved treating human cerebral microvascular endothelial cells with erythritol to assess its impact on vascular health. The results revealed that cells exposed to the amount of sweetener found in a single beverage serving exhibited higher levels of oxidative stress. Additionally, these cells produced less nitric oxide, a vital compound responsible for blood vessel dilation. Decreased nitric oxide levels can compromise vasodilation, impair blood flow, and potentially increase the risk of heart attack and stroke.

Auburn Berry, a graduate student at the University of Colorado Boulder and the lead author of the study, emphasized the need for further research to fully comprehend the implications of erythritol consumption on vascular health. While the sugar substitute is commonly marketed as a healthier alternative, consumers are advised to be mindful of their daily intake to mitigate potential risks.

The findings presented at the American Physiology Summit underscore the importance of understanding the effects of erythritol on the body, particularly its impact on brain and heart health. As research continues to uncover the potential drawbacks of this popular sugar substitute, it is crucial for individuals to make informed decisions about their dietary choices.
