President Trump’s recent signing of the “One Big Beautiful Bill Act” has set off a chain reaction in the health insurance industry, with Centene and other insurers facing challenges in managing costs for low-income Americans covered by Medicaid and individual plans under the Affordable Care Act (ACA).

Centene’s recent decision to pull its financial forecast for the year comes after discovering that subscribers in its ACA plans are sicker and require more care than initially anticipated. Additionally, the company is experiencing higher cost trends in its Medicaid business, which provides coverage for low-income Americans in over two dozen states.

Following Centene’s struggles, Molina Healthcare also lowered its earnings guidance for the year due to cost pressures in all three government-subsidized health insurance programs it manages: Medicaid, Medicare Advantage, and individual coverage under the ACA.

The future of Medicaid patients’ management is becoming increasingly complicated for insurers, as President Trump’s legislation includes $1 trillion in cuts to Medicaid and ACA plans, potentially eliminating coverage for nearly 12 million Americans over the next decade. The bill introduces more eligibility checks and bureaucratic hurdles, making it harder for insurers to retain Medicaid and ACA plan enrollees.

Analysts predict challenges in keeping Americans enrolled in Medicaid and ACA plans due to increased eligibility checks and administrative obstacles. The new legislation is expected to lead to higher premiums and reduced choices for individuals losing Medicaid eligibility.

In response to these changes, insurers like Centene may need to reprice their 2026 plans to accommodate the shifting cost profiles of ACA exchange populations. Analysts suggest that sicker patients are more likely to purchase health plans, while healthier individuals may opt out, resulting in risk pools with higher healthcare needs and increased costs for insurers.

Overall, the health insurance industry is bracing for significant changes as a result of President Trump’s recent legislation, with insurers like Centene and Molina Healthcare facing uncertainties in managing costs and retaining enrollees in Medicaid and ACA plans. The impact of these changes may lead to higher premiums and reduced coverage options for millions of Americans.
