While the SEP-1 program aims to improve the quality of sepsis care and patient outcomes, early results suggest mixed success. Hospitals have shown increased adherence to the program guidelines, with higher rates of blood lactate measurement, IV fluid administration, and antibiotic prescription. However, the overall mortality rates for sepsis patients have not significantly improved since the program’s implementation.

In an editorial discussing these findings, experts from Harvard Medical School raise concerns about unintended consequences of the reimbursement program. They note that the program may incentivize unnecessary antibiotic use in cases where the cause of sepsis is viral rather than bacterial, such as during the COVID-19 pandemic. Additionally, aggressive fluid administration may be harmful to patients with underlying conditions like congestive heart failure.
