A study published in JMIR this year supports this view, noting that the use of ambient AI scribes led to a significant decrease in documentation time during shifts (P<.001), saving about 24 minutes in an 8-hour shift when used across 20 patient interactions.
As hospitals transition from initial trials to full-scale deployment of such AI systems, new challenges are emerging that require leadership attention. Governance is crucial for ambient scribing tools as they raise concerns about the implications of having an āinvisible listenerā in the consultation room. A paper in the JMIR Medical Informatics Journal discusses potential governance issues, such as patient consent and trust, and the risk of cognitive deskilling among physicians. While patients may perceive over-reliance on AI negatively, they are also dissatisfied when doctors are preoccupied with real-time documentation instead of the conversation. Given that doctors’ workloads often donāt align with the extensive documentation required, ambient devices offer practical benefits by addressing these challenges.
Experts assert these advantages have tangible impacts on healthcare workers. A study in Cardiovascular Diagnosis & Therapy found that ambient AI tools alleviate physicians’ cognitive load, enhance job satisfaction, and boost practice-level efficiency, which could improve care accessibility. However, the study also identifies persistent issues with these tools, including documentation omissions and clinically significant hallucinations. Implementing these systems involves complex challenges, such as workflow redesign, legal considerations, and maintaining the patient-clinician relationship. In certain specialties that require precise documentation, AI errors could pose higher risks, emphasizing the need for specialized validation.
Thus, scaling these technologies is challenging. While initiating pilot projects in healthcare is relatively straightforward, implementing them broadly requires meticulous planning and change management, with stringent criteria for measuring success. As these technologies become more integrated into clinical settings, developers and leaders must remain vigilant and ready to halt deployment if patient safety or physician workflows are adversely affected.
