The new program will give doctors the autonomy to find the best way to manage chronic conditions such as hypertension, diabetes, and heart disease. Successful management will be defined by outcomes such as lowering blood pressure to normal levels, improving daily glucose determinations for diabetes patients, and avoiding hospitalizations for heart disease. By concentrating solely on improving these outcomes, primary care physicians will be able to deliver more effective and personalized care to their patients.

In terms of financial rewards, primary care doctors will be incentivized based on the number and severity of chronic conditions they manage, as well as the improved control of each medical problem. The added incentive payments will represent about 2% of total healthcare spending, equating to around $90 billion distributed across practices. This could result in a significant increase in average take-home pay for primary care doctors, potentially reaching $130,000 annually, provided they achieve the health outcome goals. This payment structure aims to bring income parity between primary care doctors and specialists who effectively manage chronic conditions.

To turn this potential into practice, clinicians will be supported by advanced technologies that can help them manage chronic diseases more effectively. Patients participating in the program will receive wearable monitors connected to generative AI tools, allowing them to track their health in real time and receive immediate feedback on managing their conditions. Educational programs will also be available to help patients make lifestyle changes that can positively impact their health.

By transforming healthcare from episodic to continuous care, physicians will be able to monitor their patients’ health more closely using technology. For example, wearable devices can collect ongoing data on blood pressure, blood glucose levels, and other vital signs, allowing clinicians to make timely adjustments to medications or treatment plans. This continuous monitoring approach could lead to earlier interventions and prevent costly hospitalizations.

The future of enhanced primary care lies in investing in preventative measures to improve patient health and lower healthcare costs. If successful, this new program could save the nation over $2 trillion by cutting the rate of healthcare inflation in half over the next six years. By preventing chronic diseases and their complications, we can achieve real improvements in both patient health and national financial stability. This program offers a promising solution that benefits patients, payers, and primary care providers alike.
