New clues emerge on how foods spark anaphylaxis

Severe allergic reactions can be swift and deadly, often triggered by insect stings, medications, or certain foods. Anaphylaxis, the life-threatening allergic reaction, can lead to swelling, difficulty breathing, and dangerously low blood pressure. Epinephrine is commonly used to treat anaphylaxis, but it doesn’t always work effectively. Researchers have been investigating ways to prevent these severe reactions before they occur.

Two new studies published in Science have shed light on a key step in the cascade of events leading to anaphylaxis. By studying mice that showed signs of anaphylaxis after repeated exposure to food allergens, researchers identified leukotrienes as crucial molecules in triggering the extreme reaction.

Leukotrienes play a role in regulating the transport of food allergens across the gut membrane to reach the bloodstream, where they can trigger anaphylaxis. Mast cells, which are protective immune cells, detect these allergens and initiate the body’s response. The researchers found that blocking the pathway regulated by leukotrienes could prevent anaphylaxis from occurring.

Stephanie Eisenbarth, an immunologist involved in one of the studies, explained that by disrupting the transport of food allergens across the gut membrane, mast cells on the other side would not be alerted to the presence of the allergen, thus preventing an anaphylactic response. This discovery led to the testing of a drug called zileuton, approved for asthma, which successfully diminished allergic reactions in mice exposed to food allergens.

The studies focused on food allergens that reach the gut; however, other allergens such as insect stings may trigger different immune responses. Injected allergens, for example, do not seem to rely on leukotrienes to activate the immune system. The research also revealed insights into the specialized populations of mast cells in the gut lining.

Despite the complexity of allergic reactions, both studies highlight the importance of investigating the role of leukotrienes in food allergen reactions.
