Major surgery can take a toll on the body, similar to running a marathon. Just as you wouldn’t attempt a marathon without training, undergoing major surgery without preparing your body for the trauma it will experience is not advisable. This is where prehabilitation comes into play. Prehabilitation involves preparing the body through exercise, nutrition, and cognitive training to better withstand surgical trauma.

Recent research published in JAMA Surgery highlights the importance of tailoring prehabilitation regimens to fit the individual needs of patients. While current prehab programs tend to be one-size-fits-all, personalized prehab regimens can reduce immune responses linked to infections and neurocognitive decline after surgery. The study conducted by researchers at Stanford University offered prehab programs to 54 adult patients undergoing major surgery, mostly abdominal procedures under general anesthesia.

Half of the patients received a booklet with exercises, nutritional tips, and cognitive training to follow for two to six weeks before surgery. The other half met remotely twice a week with specialized experts for personalized prehab sessions. The personalized sessions aimed to help patients adhere to the routine and offered customization based on individual preferences.

After prehab, blood samples were taken from each patient to map their immune system using artificial intelligence. The analysis revealed that patients who received personalized prehab had specific cellular changes, including a decrease in overreactive immune responses leading to high inflammation levels and a reduction in T cells associated with cognitive decline after surgery. The impact of personalized prehab was significantly larger compared to the standard prehab group.

The immune system changes observed post-prehab seemed to translate to better outcomes after surgery. Patients in the personalized prehab group experienced fewer moderate-to-severe post-op complications compared to those in the standard prehab group. While the study was small and larger patient populations are needed to establish a clear link between immune system responses and clinical outcomes, the findings suggest that personalized prehab can have a significant impact on surgical recovery.

Franco Carli, a prehabilitation research leader at McGill University, emphasizes the importance of understanding the biological markers and immune responses to determine the most effective prehab exercises and techniques. This personalized approach to prehabilitation can help optimize patient outcomes and improve recovery after major surgery. Prehab is not just a simple intervention; it is a comprehensive strategy to prepare the body for the challenges of surgery and promote better post-operative results.
