In a recent study published in the Journal of the American Heart Association, scientists have proposed a novel approach that combines a predictive model called PREVENT with coronary artery calcium scoring to identify individuals at high risk of their first heart attack.

The PREVENT model, developed by the American Heart Association, calculates a risk score based on traditional measures such as blood pressure, cholesterol levels, kidney function, age, and BMI, as well as additional factors like type 2 diabetes, smoking, and social determinants of health. On the other hand, coronary artery calcium scoring is done through a CT scan to detect calcium buildup in the arteries leading to the heart, which can indicate the presence of blockages and increase the risk of a heart attack.

The researchers found that combining the PREVENT score with the coronary artery calcium score provided a more accurate prediction of cardiovascular risk, especially in identifying individuals who may experience a heart attack in the near future. By analyzing the medical records of nearly 7,000 individuals in New York who underwent cardiovascular disease screening, the study demonstrated that the combined approach was more effective in stratifying risk levels compared to using either method alone.

While some experts have raised concerns about the cost and radiation exposure associated with CT scans, the study authors believe that the additional information provided by coronary artery calcium scoring can be valuable in certain clinical scenarios. For instance, individuals already taking statins may benefit from knowing their calcium score to determine if additional cholesterol-lowering medication is necessary.

The PREVENT model, which was released in 2023 as an updated version of the Pooled Cohort Equations, aims to improve the accuracy of cardiovascular risk assessment and reduce the number of unnecessary statin prescriptions. Despite the ongoing review by medical societies, the PREVENT score has shown promise in predicting cardiovascular events and guiding treatment decisions.
