When Ted Matthews decided to come out of retirement in early 2023 to take over as the CEO of Anson General Hospital in Anson, Texas, he knew he was stepping into a challenging situation. The hospital, where he had started his health care career, was struggling to stay afloat. With a dwindling patient base and financial difficulties, the hospital was on the verge of becoming the twenty-seventh rural hospital in Texas to shut down.

The situation was dire, with staff members updating their resumes and the hospital resorting to holding bake sales to cover expenses. Every board meeting was a tense affair, with decisions being made on which bills to prioritize for payment. It was clear to everyone involved that drastic measures needed to be taken to save the hospital.

In a bold move to turn things around, Anson General applied to become a Rural Emergency Hospital, a new designation created by Congress in 2023 to provide support to struggling rural hospitals. Instead of traditional reimbursement for inpatient services, these hospitals receive a fixed monthly sum from the government, amounting to over $3 million annually for Anson General. Additionally, Medicare adds a 5% bonus to their payments for outpatient services. These monthly payments, totaling around $270,000 for Anson General, have been a lifeline for the hospital, allowing it to stabilize its finances and continue serving the community.

Despite the potential benefits of becoming a Rural Emergency Hospital, only 38 hospitals have taken up this designation since its inception in 2023. Many of these hospitals, like Anson General, felt they had no other choice if they wanted to survive. The road to recovery for rural hospitals is challenging, and the decision to become a Rural Emergency Hospital is not taken lightly.

The story of Anson General Hospital serves as a reminder of the struggles faced by rural hospitals across the country. While the Rural Emergency Hospital program offers a glimmer of hope for these institutions, there is still much work to be done to ensure their long-term sustainability. With dedicated leadership and community support, hospitals like Anson General can overcome their challenges and continue to provide essential health care services to rural populations.
