The United States is currently facing a growing physician shortage, with about a quarter of doctors in practice being educated in foreign medical schools. However, newly-minted M.D.s are facing uncertainty due to a pause on new visa appointments by the Trump State department. This pause, implemented as the department develops a plan to vet visa candidates’ social media, has put thousands of students, trainees, teachers, and exchange visitors in limbo.

For foreign-born and educated doctors who have not secured a visa appointment yet, the timing could not be worse. Most medical residencies officially begin on July 1, with orientations for new M.D.s starting in June. International medical graduates without visas may miss their start date, jeopardizing their positions and leaving hospitals in a difficult position as interns play a crucial role in providing hands-on medical care under supervision.

In the main residency match in March, where graduates of medical schools and schools of osteopathy were placed into first-year jobs, 6,653 foreign-born students who graduated from non-U.S. medical schools were matched. These students must complete a U.S. residency to be licensed to practice medicine in the country, making these programs essential in maintaining the needed supply of foreign doctors.

Many international medical graduates come to the U.S. on J-1 visas, which are reserved for exchange visitors participating in teaching, research, and training. While most incoming interns have already scheduled their visa appointments, about 5%—or more than 300 doctors—have not, leaving them in a state of uncertainty.

The pause on visa appointments affects all student visa types, including F-1 visas for foreign undergraduates and graduates and M-1 visas for students studying at trade schools or non-academic programs. The State department has stated that the pause will remain in place until further guidance is issued, causing distress among those impacted.

Leadership from the Educational Commission for Foreign Medical Graduates (ECFMG) is working to gain an exception for physicians, as they serve as a gatekeeper for foreign medical graduates entering the U.S. The ECFMG has advised current residents needing to renew their visas not to travel outside the U.S. until the pause is lifted.

The ongoing uncertainty surrounding visa appointments is causing significant challenges for international medical graduates and the healthcare industry as a whole. With the physician shortage in the U.S. only expected to worsen, the timely resolution of this issue is crucial to ensure the continued flow of foreign doctors into the country.
